This form must be completed by the "Primary Carer" of the household.

Proof of Aboriginality may be requested if the carer is not known to the service.
The forms below are to be completed by all people residing in the home over the age of 18.
These forms are also to be completed by friends and family who stay in the home for extended periods of time.(e.g. an uncle who visits and stays for 3 weeks each year, needs to have a completed check)

This form allows the Hunter Aboriginal Children's Services to carry out a "Working With Children's Check".

This form is a declaration that the applicant is not a "Prohibited Person" to work with children.
As of 2nd January 2007, legislation regarding the Working With Children Check process has changed and NEW forms are now in place. Attachment 3 is now called Attachment 5 but is still the Working With Children Check Consent Form. The Prohibited Employment Declaration (Attachment 4) has also changed.
Please read these forms VERY CAREFULLY.
